Telehealth Note
Appointments are currently offered via telehealth, with in-office visits at 21035 N Cave Creek Rd, Suite 5C Phoenix AZ 85024 beginning June 1, 2026.
Primary Care Plan
$125/mo
Healthcare Membership
Includes
- 2 appointments per month
- Appointments can be for an acute illness, chronic condition treatment, pre-employment physical, or DOT physical
- Medications sent electronically to your preferred pharmacy
- Member patients have the personal phone number of their medical provider and can call or text for immediate treatment
Additional costs may include, but are not limited to, lab testing, testing supplies, bloodwork, and pharmaceuticals.
